Prior to arriving for your first appointment, please fill out the necessary paperwork below. You can either fax the completed forms to our office at (415) 373-9240 or bring them in with you to your first appointment. Please make sure to also bring in any relevant lab work or imaging studies or have them faxed to our office so we may review beforehand. If you will be using insurance to pay for your acupuncture treatments, we must verify your coverage before your first visit or you will be asked to pay the out of pocket rate until acupuncture coverage is confirmed. We are happy to verify insurance coverage, please call our office with your policy information.
